The
WCA HIV/AIDS Prevention Case Management Program
is designed to give emotional and psychosocial
support to African American women in the north
central Philadelphia community who are at risk
for exposure to HIV/AIDS. The program also
gives support to African American women living
with AIDS that are seeking to maintain their
health through an
organized case management and holistic approach.
The
following services are provided:
- Psychosocial
Assessment
- Case
Management
- Education
and Counseling
- Life
Skills Development
- Referral
& Advocacy for Utilization of Community
Resources
- Monitoring
of Internal and External Services
The
goals of the HIV/AIDS Prevention Case Management
Program are:
- To
increase the likelihood that African American
women will remain seronegative.
- To
increase the knowledge base of African Americans
living with AIDS that would enable them to
maintain their health.
- To
assist in abating psychosocial and/or cultural
issues that contribute to ill health and self-defeating
behaviors.
- To
strengthen the family unit.
- To
attain self-sufficiency.
